How the math works
Three conversions, nothing more
01 · Concentration
mg ÷ mL
Dissolving a known amount of powder in a known volume of water gives a concentration. 5 mg of powder in 2 mL of water is 2.5 mg/mL — the powder is simply spread evenly through the liquid.
02 · Volume
dose ÷ concentration
A dose is a fraction of the vial, so it is the same fraction of the liquid. At 2.5 mg/mL, a 250 mcg (0.25 mg) dose corresponds to 0.1 mL of solution.
03 · Units
mL × syringe scale
Insulin syringes mark volume in units. On a U-100 syringe, 1 mL = 100 units, so 0.1 mL reads as 10 units. The same 5 mg vial holds 20 such 250 mcg doses (5000 ÷ 250).
